What you'll need for First Time Passport Applications:

1. Form DS-11 (Click Here for information & the form http://travel.state.gov/passport/get/first/first_830.html )

2. Certified Birth Certificate (must be official with the raised seal)

3. Proof of Identity (Driver's License, State ID)

4. You will also need a Passport Photos, the following places can take your picture for the passport; Most Post Offices (check with your local office) Kinkos, Walgreens, AAA and The UPS Store.

**Please note that obtaining a passport can take up to 8 weeks**
